Faster, Cleaner SOAP Notes: 6 Dictation Habits That Save Time

Most of the time you spend cleaning up a dictated note isn't the AI getting things wrong — it's the note capturing the visit in the order you happened to talk through it, rather than the order a SOAP note expects. A few habits fix most of that before you ever hit "Format."

1. Say the section out loud as you move through it

You don't need to announce "Subjective" or "Objective" like a script, but pausing briefly when you shift from history to physical exam findings gives the model a clean seam to work with — fewer findings end up misfiled.

2. Read numbers back deliberately

Weights, temperatures, and drug doses are the one place where a fast mumble costs you real editing time later. Slow down for anything numeric — it's the fastest single fix for accuracy.

3. Note the plan even if it feels obvious

"We'll recheck in two weeks" takes two seconds to say and saves you from adding it manually afterward. Anything you'd normally tell the client, say it — it usually belongs in the note anyway.

4. Use the patient history panel before you start

If it's a returning patient, glance at prior visits before you dictate. You'll naturally reference what's changed instead of re-describing baseline findings from scratch.

5. Don't over-edit the first draft

It's tempting to rewrite phrasing to sound more like "you." Skim for accuracy first, then decide if wording actually needs to change — most of the time it doesn't.

6. Even a quick dictation carries over to Analysis

You don't need a full visit note for this to pay off. If you simply dictate the signalment and reason for the visit, that information is captured too — send it to Analysis afterward and those details are already filled in, instead of retyping them from scratch.
